equipment malfunction, weather, or some other physical impediment and there is no fault or negligence of customer.

-For Tax exempt, provide BIR Form 2303 & Certificate of Tax Exempt. Otherwise, 5% EWT to be deducted with BIR Form 2307 (Certificate of Creditable Tax Withheld) upon payment.
